The USMLE Step 3 examination assesses whether you can apply medical knowledge and understanding of biomedical and clinical science essential for the unsupervised practice of medicine, with emphasis on patient management in ambulatory settings. The Step 3 exam provides a final assessment of physicians assuming independent responsibility for delivering general medical care. The examination is organized along two principal dimensions: clinical encounter frame and physician task. Most recently, two new sections have been added: a general principles section and a section on immune disorders. The general principles section includes a description of medical ethics and jurisprudence.
The purpose of Step 3 is to determine if a physician possesses and can apply the medical knowledge and understanding of clinical science considered essential for the unsupervised practice of medicine, with emphasis on patient management in ambulatory care settings. Step 3 emphasizes selected physician tasks, namely, evaluating severity of patient problems and managing therapy. Assessment of clinical judgment will be prominent. Clinical problems encountered on the examination involve mainstream, high-impact diseases. Provision is made for less common but important clinical problems as well. Test items and cases are patient centered, starting with a description of a clinical encounter (vignette). Both the multiple-choice items and case simulations pose action-related challenges that require clinical decisions or judgment. Emphasis is on ambulatory patient encounters; however, inpatient encounters of significant complexity and reflecting contemporary trends also are represented. Provision is made for incorporating applied basic and clinical science concepts, especially as they relate to justification for prognosis or management.
Some of the tasks in the examination may include:
• Initial workups, which stress the initial assessment and management of patients whom the physician is seeing for the first time; the range of possible clinical problems that may be presented is broad and may include undifferentiated symptoms, chronic illnesses, and emotional problems
• Continued care, which stresses the continuing management of previously diagnosed diseases
• Urgent care, which stresses the management of life-threatening emergencies
These encounters may occur in a variety of practice settings: a satellite health center, a general practitioner’s office, an emergency department, and an inpatient facility.
Furthermore, the examination now focuses on a core group of physician tasks that the test taker must master. These tasks include:
• Obtaining a history and performing a physical examination
• Using laboratory and diagnostic studies
• Formulating a likely diagnosis
• Evaluating the severity of a patient’s problems
• Managing patients, including health maintenance, clinical intervention, clinical therapeutics, and legal or ethical issues
• Applying scientific concepts
This review book is designed to be an examination preparation guide modeled after the USMLE Step 3 examination content. It exposes the reader to all of the practice settings and physician tasks tested on the examination. Although we cannot provide patient simulations as on the examination, this review guide emphasizes the main tasks and concepts that will be tested on the actual examination. Topics of discussion include relevant subject material in internal medicine, general surgery, psychiatry, pediatrics, neurology, and obstetrics/gynecology.
The book is divided into six composite examinations, with detailed explanations of the answers to the multiple-choice questions. The questions and explanations can be used in several ways:
• As a diagnostic tool to assess competency in particular subject areas and identify areas in
which further review is needed
• As a postreview test to assess whether core material has been mastered
• As a primary means of review
We believe this book provides a comprehensive review of the relevant topics expected to be mastered on the USMLE Step 3 examination, presented in a format that maximizes learning.
